Two people were killed and three others were injured Monday in a three-vehicle accident on U.S. Highway 62 east of Green Forest.

Madonna Woodward, 34, of Berryville and a 16-year-old boy were killed, according to a preliminary report from the Arkansas State Police.

According to the report by ASP Sgt. Chad Hipps, Woodward was driving a 2002 GMC Envoy eastbound at noon Monday when it crossed the double yellow lines and struck a westbound 2002 Chevrolet Tahoe driven by Rigoberto Tovar-Tellez, 34, of Berryville. The Envoy then spun in the roadway and struck a westbound 2006 Honda Pilot driven by the 16-year-old boy.

Tovar-Tellez and a 7-year-old girl who was a passenger in his vehicle were injured, according to the report. Hector Calderon-Moreno, 66, a passenger in the Honda Pilot, also was injured.

The road was dry and the weather cloudy at the time of the accident, according to the report.

The two fatalities brought the total number of people killed in accidents on Arkansas roadways in 2018 to 435.
