Alexander (Alex) John Phillips was born Sept. 3, 1990 in Springfield, Mo. to Michael and Tabatha (Fultz) Phillips. He departed this life Wednesday, Apr. 13, 2016 in Rogers, Ark. at the age of 25.
Alex attended Berryville Schools, Easter Seals facility, and other organizations. He liked the Razorbacks, race cars and time spent with family and friends.
He is survived by his father Michael Phillips and Sheila of Shell Knob, Mo.; his mother, Tabatha Simmons of Berryville; one sister, Samantha Chaney of Eureka Springs, Ark.; his brother, Kolton Simmons of Judsonia, Ark.; step-brother, Brendon McConnell of Berryville; grandparents, James and Carolyn Fultz of Berryville; aunts and uncles, Pam and Danny Fancher of Shell Knob, Melissa and Jerry Luttrell of Kansas City, Mo., Allen Phillips of Hot Springs, Johnnie Phillips of Berryville, Angela Marsee of Rogers, Melodie and Joe Payne of McRae, Ark., Brandon and Amy Fultz of Berryville, Ark. and a number of cousins and many friends.
He was preceded in death by his grandparents, John and Shelby (Davis) Phillips.
Alex was loved by all who knew him and was known for his big, heart-warming smile. He loved all his caregivers, they were very special to him.
A memorial gathering to celebrate his life will be held from 2-4 p.m. on Sunday, April 17, 2016 at Smith Family Funeral Home in Green Forest, Ark. A private family burial will be at a later date. On line condolences at www.smithfamilyfuneralhome.net.
